An EHR built for agents, not just clinicians
Most electronic health records were designed as digital filing cabinets and later had automation bolted on. The consequence is familiar to anyone who has integrated with one: data locked in proprietary structures, interfaces built for human clicking rather than programmatic access, and AI features that sit beside the workflow instead of inside it.
XEHR starts from the opposite premise. If autonomous agents are going to perform administrative work, the record they operate on must be legible to them — standards-native, API-first, and structured so that an agent can read a chart, act on it, and write back an auditable result without screen-scraping anything.
FHIR R4 native
Clinical data is modelled as FHIR resources rather than exported into them on request. Interoperability becomes a property of the system rather than an integration project, which matters for both agent access and for the data-sharing obligations practices now operate under.
USCDI data elements
The United States Core Data for Interoperability defines the minimum dataset a certified record must support, covering demographics, vital signs, laboratory results, medications, allergies and clinical notes. Supporting it as a first-class structure is what makes downstream automation reliable.
Practice management in the same system
Scheduling, registration, eligibility and patient communication share a data model with the clinical record. The alternative, an EHR bridged to a separate practice management system, produces exactly the reconciliation gaps that administrative automation then has to work around.
Revenue cycle as a native capability
Charge capture, coding support, claim submission and denial management operate on the same record as the encounter that generated them. A code proposed from a note can cite that note, and a denial can be traced to the documentation that caused it.
Built for agent access
Agents authenticate as scoped identities with least-privilege permissions, and every read and write they perform is attributed and logged. This is what allows autonomous work to be audited afterwards with the same rigour as human work.
HIPAA controls throughout
Encryption in transit and at rest, role-based access control, immutable audit logging and defined retention policies apply uniformly, including to the automated components. Security that stops at the boundary of the AI features is not security.
Why the architecture determines the ceiling
The limiting factor on healthcare automation is rarely model capability. It is access: whether the relevant data can be read reliably, whether an action can be written back safely, and whether the whole exchange can be audited afterwards.
A record designed around those three requirements raises the ceiling on everything built above it. One designed around human data entry, with automation retrofitted afterwards, caps what any agent can do no matter how capable the underlying model becomes.
XEHR is live today
XEHR is in production. It combines AI clinical documentation, autonomous revenue cycle agents, FHIR R4 and HL7 native interoperability, and HIPAA and SOC 2 readiness in a single platform built for US practices, and it is the system RCM Employee works inside when a practice runs both.
For practices that have outgrown their current software, we also build EHR and practice management systems from the ground up — specialty-specific charting and templates, lab, pharmacy and clearinghouse integrations, role-based access with a full HIPAA audit trail, and migration from Athena, eCW, Kareo, NextGen and Practice Fusion. For practices keeping their existing EHR, we add production-grade X12 EDI so claims, eligibility and remittances flow without manual workarounds.
Questions we get asked most
What makes an EHR AI-native rather than AI-enabled?
An AI-enabled record adds features such as note summarisation to an architecture designed for human data entry. An AI-native record is built so autonomous systems are first-class users: standards-based data models, API-first access, scoped machine identities and audit logging that attributes every automated action.
Why does FHIR R4 matter?
FHIR R4 is the standard US healthcare data exchange has converged on. A record that stores data as FHIR resources natively can exchange it without a translation layer, which removes both the cost of custom interfaces and the data loss that translation between proprietary models tends to introduce.
Does XEHR replace our practice management system?
It combines the clinical record and practice management in a single system with one data model, so scheduling, registration, eligibility and billing operate on the same underlying record. Practices running separate systems today can evaluate it as a consolidation rather than as an additional integration.
How is patient data protected?
Through the controls HIPAA requires of any system processing protected health information: encryption in transit and at rest, role-based least-privilege access, immutable audit logging, defined retention and disposal policies, and Business Associate Agreements covering every processor. These apply to automated components on the same terms as human users.
Can it exchange data with hospitals and other practices?
That is the purpose of building on FHIR and USCDI. Standards-native storage is what makes participation in interoperability frameworks and query-retrieve exchange with other organisations practical rather than a bespoke project for each counterparty.
What size practice is this designed for?
The architecture targets independent and mid-sized practices, which carry the same regulatory and administrative burden as large health systems without the staff to absorb it. That group has the most to gain from consolidating the clinical record, practice management and revenue cycle into one automatable system.
